Friends For Life – Position Available – Medical Case Manager
SUMMARY:
The Friends For Life Corporation Medical Case Manager will ensure that consumers are provided with a range of client center services, including treatment adherence, linkage with health care, psychosocial, insurance coverage and other supportive services. Coordinating and follow up of medical treatments is a component of Medical Case Management. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems. This includes all types of case management including face -to-face, phone contact, and any other forms of communication.
EDUCATION and/or EXPERIENCE:
A Bachelor level degree in a health or human services related discipline with equivalent to two year of full time professional case management in a public service agency. or – A Bachelor level degree in Social Work with equivalent to two years full time professional case management in a public service agency (an appropriately supervised BSW internship may count as one year’s experience) –or-Master level degree in a health or human services related discipline with the equivalent to one year of full time professional case management in a public service agency. Experience working with persons with HIV/AIDS preferred. Must have a valid State Driver’s License, provide proof of automobile insurance coverage to employer at all times and have an automobile to be used daily to perform job duties. Must be able to pass a drug screen and background check.
