Friends For Life – Position Available – Medical Case Manager

SUMMARY:

The Friends For Life Corporation Medical Case Manager will ensure that consumers are provided with a range of client center services, including treatment adherence, linkage with health care, psychosocial, insurance coverage and other supportive services. Coordinating and follow up of medical treatments is a component of Medical Case Management. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems. This includes all types of case management including face -to-face, phone contact, and any other forms of communication.

EDUCATION and/or EXPERIENCE:

A Bachelor level degree in a health or human services related discipline with equivalent to two year of full time professional case management in a public service agency. or – A Bachelor level degree in Social Work with equivalent to two years full time professional case management in a public service agency (an appropriately supervised BSW internship may count as one year’s experience) –or-Master level degree in a health or human services related discipline with the equivalent to one year of full time professional case management in a public service agency. Social Networks Testing Strategist SUMMARY:

MEMPHIS, TN – Planned Parenthood Greater Memphis Region is offering a series of monthly “lunch and learn” training sessions on a variety of topics involving reproductive health care, sexual health education and HIV/STI prevention. These trainings are designed for medical professionals, staff, volunteers and interns at agencies working in reproductive health, family planning, STI or HIV prevention, etc.

All trainings take place at Planned Parenthood’s offices at 2430 Poplar Avenue, from 11:00 a.m. to 1:00 p.m. The trainings are free and lunch will be provided for training participants, but participants must register prior to the date of training.

Below is the list of training dates and topics. Participants may register for one class or the entire series of classes:

1. Provides HIV testing, counseling, and RESPECT counseling to PPGMR’s Hispanic Outreach and Health Center clients.

2. Runs a network of HIV testing community locations, including locations that require weekly after-hours testing.

3. Cultivates and maintains relationships with agencies and service providers in the community with a particular focus on creating opportunities to provide outreach education and HIV testing to Spanish-speaking clients.

4. Supports staff in creating educational materials and experiences linguistically and culturally appropriate for Spanish-speaking clients

5. Attends training programs, seminars, conferences, and other meetings relating to the delivery of HIV related services

6. Maintains accurate records of HIV testing, client care and referral, education, and outreach activities.

Job Qualifications:

Education: Bachelor’s degree in health, education, counseling or related area preferred, fluency in Spanish required.

Experience: 1-2 years of experience in health promotion with Spanish-speaking clients strongly preferred; sexual and/or reproductive health experience preferred Skills: Ability to work independently and communicate effectively with youth, parents, community leaders, facilitation, organization, program development, and marketing skills.

Oversee grant contracts to ensure timely earning of contracts. Maintain open communications with grant funders. Review all contracts for accuracy prior to execution. Maintain all grant and contract information in FR50. Complete reports as required that show annual levels of grants submitted and funded. Maintain electronic file of all grant requests, proposals and contracts and other grant/contract related materials. Other duties as assigned.

Evaluation

Take lead in ensuring agency-wide evaluation program is maintained. Ensure all program assessments are completed on schedule, data entered into CAREWare and program reports completed on schedule. Update evaluation forms as necessary. Complete all reports necessary to maintain up-to-date evaluation data. Analyze all data to ensure proposed program outcomes are being met. Provide accurate, complete and timely data entry and reporting of all relevant program and consumer data into CAREWare, HMIS, Evaluweb and/or other reporting systems as requested by supervisor.

Manage all aspects of the Friends For Life website. Create, write and distribute an agency e-newsletter monthly.

1. Works with Epidemiology staff to ensure availability of accurate, up to date, complete, comprehensive and

timely census and HIV/AIDS/STD/TB/HEP data on epidemiological and demographic indicators – collected,

collated, analyzed and interpreted at levels. Identifies other data that relates to the project and include in analysis

as appropriate (e.g., number of persons receiving HIV/AIDS drugs, HIV + person in TGA jails, HIV + persons in VA

system, State Medicaid HIV data).

2. Coordinates data activities with primary partners (MSCHD and State Epidemiology staff).

3. Identifies other entities that have relevant data and develop/maintain those relationships.

4. Leads and participates in the development of the TGA needs assessment by planning and implementing

assessment of HIV/AIDS related community resources and needs including organizing, conducting and analyzing

results of focus groups.

5. Designs, supervises and analyzes results of surveys; collects and analyzes research data related to HIV

services within the TGA.

6. Actively participates and contributes the Memphis TGA Comprehensive Plan.

7. Designs and implements special studies to trend and review key aspects of the project (service utilization

patterns, special population studies, access to care measurement) and complete special reports.

8. Presents data and reports to stakeholders (e.g., Planning Council, Ryan White staff) through written, electronic

9. Monitors and provides oversight of data collection from contractor and sub-contractors and serves as a point of
